Details: The Patient Navigator will establish contact with cancer patients within the hospital setting through approaches agreed upon with the host site to raise awareness about American Cancer Society information resources, support programs and services, and assist in meeting other cancer-related needs articulated by patients and families. Responsible for establishing an ongoing relationship with the health care community and the host location of the ACS Patient Navigator program. Helps to enhance and maintain the Cancer Resource Connection. Some travel may be required.Work with hospital cancer patients to assist them in accessing the hospital’s cancer care delivery system and ACS resources. This will include direct interaction with patients and their families in person, over the phone and by email. Creates, with families, service plans for families and children that are inclusive of stakeholders’ input; build on client strengths; that are realistic, culturally competent, provide meaningful assistance, attend to the child’s mental and behavioral health, and are accordance with City, State and Federal Regulations.  3. Ensures that children are safe and well cared for in the foster home through development and maintenance of positive working relationships with the child and foster parents, regular contact with child/foster parent, monitoring of the child’s behavior plan, and immediate response to early signs of disruption. 4. Attends to the child’s need for supportive and therapeutic services by ensuring that each child is actively engaged in treatment, structured recreation, mentoring, and other programs as indicated. 5. Gauges parents’ progress in achieving service goals through careful observation of parent/child interaction at visits, feedback, from other service providers, home visits and interviews. Works with families to adapt service plan throughout the life of the case. 6. Explores kinship resources for all children 7. Ensures that each child is meaningfully engaged in their education 8. Ensures that each child is progressing towards permanency with the goal of achieving permanency within a year. Recommends and prepares for TPR, ICPC, surrenders, adoptions, custody arrangements, etc., as needed. 9. Works collaboratively and communicates clearly and respectfully with colleagues, families, and other service providers. Ensures full and timely written and verbal communication with families, colleagues, ACS, B2H, and other stakeholders on case developments.10. Regularly appears in Family Court hearings related to the children and families served.  11. Supervises family/sibling visits both at the agency and off site 12. Writes timely, clear and concise progress notes, FASPs, Permanency Hearing Reports documenting child and parent progress, and attending to the uniqueness of each specific child. Writes other documents as required.  13. Attends weekly supervision meetings to report on cases and presents for discussion any problems, difficulties, or unresolved issues. 14. Participates in staff meetings, seminars, and training. Maintains and applies a current knowledge of internal and external regulations and policies applicable to foster care. 15. Attends & plays an active role in Family Team Conferences (FTC’s), 20 Day Conferences, Parent to Parent Conferences, and Cluster Meetings. 16.
