Intake Therapist
Details: Eyerly Ball CMHS is a non-profit agency in Des Moines and Ames, IA that provides services to adults with mental illness. Eyerly Ball's outpatient clinic is in the process of implementing an Open Access model and is seeking two (2) full-time Intake Therapists. POSITION PURPOSE & GENERAL DESCRIPTION: To assess and coordinate the initial intake and assessment of consumers at the Eyerly Ball Outpatient Clinic, to ensure the most appropriate level of care in accordance with all state and federal regulations as well as agency policy and procedures. This includes provision of crisis assessment and support to consumers and family members. ESSENTIAL FUNCTIONS: Participate in the Comprehensive Assessment process with each consumer and utilize this information in the development of a cohesive and comprehensive written assessment and an initial individualized treatment plan, which will be in effect for the first 30 days of agency service. 2. Initiate Consumer Engagement Standards as defined below: a. Provider each customer with a tentative diagnosis b. Facilitate the development of an initial treatment plan c. Establish an Episode of Care that defines the treatment process in recommended appointment types, frequency and duration of treatment process. 3. Provide an accurate and detailed Diagnostic Review (utilizing the DSM IV TR), based on face-to- face interview with the consumer/family, the current and past CMHC medical record and other documentation provided by the referral source or consumer/family. 4. Document well and in a timely manner, the service that has been delivered and complete required/requested external and internal reports and other paperwork in a timely manner. 5. Ensure that all billing and recording functions are conducted in compliance with all ethical and legal standards. 6. Work with the Management to develop and implement specialized programming for target populations. Provide consultation, education, and training when requested and as needed. 7. Maintain consumer and program records for consumers to ensure that the services are provided and documented in accordance with Medicaid Standards. 8. Meet billable hours standards, as determined by administration. 9. Assist in referring or coordinating community resources identified for the consumer. 10. Refer to and facilitate other treatment interventions/supports provided by other EMCMHS team members that are indicated therapeutically. 11. Cooperate with payor sources/managed care companies in providing needed information. PROFESSIONAL ACCOUNTABILITY: 12. Support and abide by departmental and agency policies and procedures as well as adhere to ethical standards applied by accrediting agencies, licensing boards and the Eyerly Ball CMHS Code of Ethics. 13. Mental Health Therapist - Licensed Social Worker
Details: Eyerly Ball Community Mental Health Services is a growing non-profit agency that serves adults with mental illness. The agency will begin providing services in Story County and will be starting up another successful Outpatiet Clinic in the Story County/Ames area. We are seeking Therapists/Licensed Independent Social Workers to be a part of the new and exciting addition of services. The position purpose will be to provide behavioral health services to consumers through a variety of modalities. To assess the consumer’s strengths, mental health and needs, then develop and implement an individualized service plan designed to restore the consumer’s optimum level of functioning. ESSENTIAL FUNCTIONS: 1. Through the intake process, evaluate the need for treatment by assessing the consumer’s strengths, needs and mental status. 2. THERAPIST
Details: Bilingual in English / Spanish a must The Staff Therapist is primarily responsible for working collaboratively with bachelor level case managers and family advocates to provide comprehensive treatment services to children, adolescents, and their families. ESSENTIAL JOB FUNCTIONS: Provides direct clinical services to clients and their families, including individual, group and family treatment. Provides case management services as needed, which include but not limited to: Monitoring clients in the Medication Support Service Program and referrals to outside or internal resources, as needed. Collaborates with other professionals both within the agency and outside the agency, as appropriate. Responsible for case documentation, written reports, and correspondence with other agencies involved with the client. Shares responsibilities for on-call emergency consultation and crisis intervention, including after-hours telephone coverage. Attendance at agency staff meetings, case consultations and in-service training. Other duties as assigned by Service Director, within recognized scope of practice. Must meet productivity standards.
