Details: ESSENTIAL DUTIES & RESPONSIBILITIES1. Demonstrate appropriate judgment skills to be able to make independent clinical decisions in the care of the post medical hospitalized recipient’s. 2. Remain flexible and responsive when changes occur in recipient’s activity and workload. 3. Demonstrates knowledge of therapeutic action, side effects and interactions of medications. 4. Evaluate/assist in evaluation of the recipient’s on an ongoing basis with communication to all disciplines. 5. Acts as a comprehensive medical care coordinator for recipient’s, assessing the needs accordingly and facilitates communication amongst other clinicians providing care. 6. Demonstrate the knowledge and possess the ability to assess data reflective of the recipients needs. 7. Interpret the appropriate information needed to identify each recipient’s requirements relative to his or her age-specific needs. 8. Develop and implement a realistic Plan of Care for each recipient, evaluate and revise this plan as needed. 9. Compare assessment data to baseline assessment to monitor the recipient’s progress. 10. Recognize alterations in function, including life-threatening situations. Intervene and document accordingly. 11. Advise all agencies involved of the changes to the treatment plan or course of treatment. 12. Place the needs of the recipient’s first at all times. Medical Care Management and service delivery are developed with the goals of the recipient healing and support in mind. 13. Encourage a positive environment and experience for co-workers and recipient. 14. Maintain confidentiality of all CHOICES and consumer related information. Protect recipient’s rights as defined in the patient Bill of Rights. EDUCATION & QUALIFICATION REQUIREMENTS: •Graduate of an accredited School of Nursing, College or University • Must possess at least one of the following combinations of education & experience: Baccalaureate Degree in Nursing (BSN) or a health or human services field; OR Associates Degree or Diploma in Nursing plus two years of certified health care nursing experience. One (1) year of Nursing Case Management preferred. • Currently licensed and registered to practice with the State Board of Nursing. • One (1) year experience in medical/surgical nursing required. • Current BLS certification. • Demonstrates leadership, organization and communication expertise. • Excellent verbal and written communication skills. • Ability to operate a computer and/or a tablet PC. • Ability to travel day-to-day within assigned geographic areas.
