Patient Navigator,Cleveland Hospital (BilingualSpanish/English)

Details: The Patient Navigator will establish contact with cancer patients within the hospital setting through approaches agreed upon with the host site to raise awareness about American Cancer Society information resources, support programs and services, and assist in meeting other cancer-related needs articulated by patients and families. Responsible for establishing an ongoing relationship with the health care community and the host location of the ACS Patient Navigator program. Helps to enhance and maintain the Cancer Resource Connection. Some travel may be required.Work with hospital cancer patients to assist them in accessing the hospital’s cancer care delivery system and ACS resources. This will include direct interaction with patients and their families in person, over the phone and by email. Implement new policies and/or procedures within the functional area to support the continuous improvement of operational effectiveness and efficiency. Hold self and others to high standards in correctly collecting constituent data, maintaining records and recommending services. Submit service requests within 48 hours of request. Provide follow-up with patient 2 weeks following initial interaction. Maintain high level of interpersonal skills to handle sensitive and confidential situations. Position requires strong ability to maintain poise, tact, and diplomacy. Respond to inquiries and interpret policies and programs to the patients and appropriate medical staff, including individuals at all levels of the ACS and to the public. Evaluate, develop, market and implement collaborative initiatives for patient population to promote supportive care programming. Make business based decisions and represent the organization in a professional business manner. Proactively and assertively addresses issues as they arise. Make effective presentations to appropriate audiences. Apprise supervisor of status of key topics of work and provides viable in innovative solutions to problem.Develop and manage partnerships with key staff, collaborating partners, staff members, and/or constituents for the completion of all duties and responsibilities of the position. Adapt quickly to multiple communication approaches and ensure two-way collaborative communication as vital to building positive relationships.Work collaboratively with Administrator/site liaison at hospital work site. Collaborate with hospital medical center staff to provide the Society resources and support as well as hospital services to appropriate patients. Initiate conflict resolution that supports both ACS business standards and maintains the partnership with the key staff or volunteer. Provide input to goals and objectives. Attend and contribute to meetings/trainings. Compose correspondence and responses to correspondences on with little or no oversight, sometimes of a complex nature.Ensure that all data maintained within operational database is up-to-date, accurate and comprehensive. Ensure proper coding of program delivery in Siebel. Ensure confidentiality for all patient files. Adhere to American Cancer Society and HIPAA regulations in handling confidential information. Report any breeches in confidentiality according to Division protocol.Ensure ACS branding standards and image are incorporated into all initiatives. Ensure timely and accurate input to and application of systems, including but not limited to: mission scorecard, Connections database, volunteer tracking systems, Siebel, Ariba, expense management, (P Card, T&E), lotus notes, (mail calendar), activity tracking. Undertake related duties as assigned and completes duties commensurate with policies and procedures of Employee Handbook. Accurately and effectively, represent the ACS and its mission, always acting in the best interest of the Society. Develop and maintain business understanding of program and event offerings of the Society to enhance the image of the ACS in the public eye. As requested, complete continuing education programs offered at the Regional, Divisional, and National level. Assist in maintenance of accuracy and completeness of the position description.
