Details: POSITION SUMMARY: Facilitate and work with consumers on a variety of activities of daily living (ADLs) needed for consumers to become more self-sufficient and move toward recovery. Additionally, you will facilitate group therapy, manage intakes/discharges, monitor medications, create a safe environment for the consumers, help coordinate treatment, and advocate for individuals with severe and persistent mental illness. Responsible for direct service care to consumers as part of a multi-disciplinary team. ESSENTIAL JOB FUNCTIONS: 1. Provide individual psychotherapy with consumers to provide support and insight.2. Facilitate group therapy sessions within the house.3. Coordinate clinical care with case management services and other services both inside and outside of MHCD.4. Details: Will implement the daily operation of the assigned residence. Will implement ISP/IEP /Treatment Team goal methodologies and data collection.Will maintain mandated OPW/DD medication certification and will dispense medication as assigned. Will be prepared for and attend ITP and Treatment Team meetings. Ensures implementation of Treatment Team procedure. Will act as shift leader in the absence of the Residence Manager. Will be required to maintain van certification.
