The Clinical Social Work Supervisor works as a partner with the RN Case Manager in identifying patient needs, barriers to care progression along with developing and executing timely discharge plans. Serves as a social work resource to the hospital and case management staff, provide support and direction with complex cases as needed. Supervises the hospital’s social workers and continually updates the Director of Case Management. Requirements Must have 5 Years of experience in a hospital setting. MSW degree from a school of social work accredited by the Council of Social Work Education. Current California LCSW license issued by the Board of Behavior Sciences required. Details: Summary: Provide direct mental health clinical assessments and treatment to the persistant and severe mentally ill, including evaluations, monitoring treatment plans, group and individual therapies. Targeted population for this position is adults. Essential Job Functions Provide psychological evaluations/problem identification, crisis interventions, and individual, group, and/or family therapy to assigned clients. Provide case management services to clients as needed. Develop treatment plans; monitor treatment progress, and follow-up at disposition times. Assist clients with psychological development, social development, and rehabilitation. Coordinate client's use of other Corporation programs and outside community resources. Coordinate medication/medicinal needs of clients with medical sources. Maintain accurate and timely clinical records consistent with Corporation standards. TDF is currently seeking a results driven individual to serve as Director of Supportive Services and Quality Assurance. Reporting to the Executive Director of Programs, the Director of Supportive Services and Quality Assurance will be responsible for overseeing The Doe’s Fund’s supportive services and providing quality assurance throughout its programs. The position is also responsible for report writing and the development of policy.
