POSITION SUMMARY: Selected candidate will provide ongoing advocacy and support for adult Day Treatment & IRA program consumers, ensuring that their rights and dignity are upheld on a daily basis. Regularly acting as a psycho-educational resource and source of support for consumers based on their individualized needs. Providing counseling and training on an ongoing and as-needed basis. Effectively communicating with consumers’ families/correspondents to facilitate their participation and input in the treatment planning process. Assessing the consumers’ current situation to facilitate necessary contacts between various parties to ensure that appropriate services are rendered.
