Details: POSITION PURPOSE AND GENERAL DESCRIPTION: Serve as a member of the Forensic Assertive Community Treatment (FACT) multidisciplinary team to assess, plan, develop, coordinate and provide treatment, rehabilitation, and support services to program consumers with severe and persistent mental illness under the clinical supervision of the Team Leader and psychiatrist. Maintains key working relationships with Magellan and Polk County Health Services, other providers in the service network, consumers and families of consumers. Function as a clinical member of the multidisciplinary team and provide treatment, rehabilitation, and support serves or may be designated by the Team Leader to function as the lead mental health professional, lead registered nurse, a team vocational specialist or substance use specialist. ESSENTIAL FUNCTIONS: 1. Take the lead role or participate in providing medication administration and medical services. a) Under the direction of the team psychiatrist and in collaboration with the other registered nurses on the team, develop, revise, maintain, and supervise team psychopharmacologic and medical treatment and medication policies and procedures including transcribing, administering, evaluating, and recording psychotropic medications prescribed but the team psychiatrist. b) Evaluate and chart psychotropic medication effectiveness, complications, and side effectsc) Arrange for required lab work according to protocol d) Organize and manage the system of getting medication to clients and integrate medication into client’s individual treatment plans. e) Manage pharmaceuticals and medical supplies. f) Coordinate, schedule, and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment with medial treatment. g. Administer IM injections according to protocolh. Properly destroy medications according to protocoli. Follow up on orders, lab results and medication prescriptions. j. Schedule client appointments with psychiatrist. 2. Provide case management for an assigned group of consumers. Ensuring no lapse in services (i.e. Medicaid/Medicare, food stamps, housing, transportation, chore services, home health, etc.). Providing practical help and supports, advocacy, coordination, side-by-side individualized support, problem solving, direct assistance, training and supervision to help consumers obtain the necessities of daily living, including medical and dental health care; legal and advocacy services; financial support such as entitlements or housing subsidies; supported housing; money management services and transportation. 3. Assume primary responsibility for developing, writing, implementing, evaluating and revising treatment goals and plans for an assigned group of consumers, within FACT policies and procedures. Taking lead role or participating in providing substance use and treatment services, the provision of rehabilitative services, providing activity of daily living services, assisting with social and interpersonal relationships and leisure time. 4. Conduct comprehensive assessment of psychiatric history, mental status and diagnosis, physical and dental health, use of drugs and alcohol, education and employment, social development and functioning, activities of daily living and family structure and relationships. 5. Document consumer progress in a timely manner, as established by FACT policies and procedures, to maintain a permanent record of consumer activity according to established methods and procedures. (i.e. progress notes, treatment plans, assessments, release of information, authorization for services, client change forms, face sheets, etc.) 6. Responsibly perform shift management duties according to established policies and procedures. 7. Provide on-call crisis intervention covering nighttime hours and serve as backup to evening and weekend staff. 8. Act as a liaison and consult with community agencies and families to maintain coordination in the treatment process. 9. Educate and support consumer’s families and advocate for client’s rights. 10. Assist consumers in building supportive networks; establishing and maintaining connections and positive relationships. 11. Take lead role or participation in the development, implementation, evaluation or revision of group opportunities (i.e. socialization, chemical dependency, emotional management, illness management and recovery, nutrition, etc.) 12. Assist consumers, their families and guardians in maximizing their abilities for self-direction, by empowering them to the greatest extent possible and coordinate community resources identified for the consumer, family members, and/or providers. 13. Mediate conflicts related to consumer services between providers, parents, consumers, and/or the agency relative to the identified needs of the consumers. 14. Professionally and effectively present information and respond to questions from consumers, medical professionals, management staff, and the public. 15. Maintain a high standard of courtesy and cooperation in dealing with co-workers, consumers and visitors while demonstrating good teamwork skills including problem solving, cooperation, stating opinions and negotiating. 16. Arrive on time and work as scheduled. 17. Provide crisis intervention, as needed. PROFESSIONAL ACCOUNTABILITY 18. Support and abide by departmental and agency policies and procedures as well as adhere to ethical standards applied by accrediting agencies, licensing boards and the EB Code of Ethics. 19. Perform job in a safe manner, insuring that safety for self, co-workers and consumers is of the highest priority. 20. Maintain consumer and employee confidentiality, abiding by all state and federal confidentiality expectations including those established by HIPAA. 21. Perform other duties as assigned.
